چکیده

Background: Though many guidelines about nutritional support, there are still dispute about permissive underfeeding, especially when the protein supply was enough. We performed a systematic review and meta-analysis to compare underfeeding with standard feeding on the clinical outcome of ICU patients. Methods: Data from the MEDLINE, EMBASE, Cochrane database, Web of Science and https://clinicaltrials.gov/ (until December, 31, 2016) were searched to identify relevant studies. Trial sequential analysis (TSA) was also calculated. Results: A total of 6 studies with 1455 patients were included in the analysis. Overall, there was no significant difference in the risk of mortality [RR=0.82, 95% CI (0.59, 1.14)] and the length of ICU stay [Mean Difference=-0.94, 95% CI (-5.57, 3.69)] in the permissive underfeeding group with standard feeding group. The incidence of nosocomial acquired infections also had not significant difference in the two group [RR 0.95; 95% CI (0.83, 1.09); p=0.46]. Conclusion: This meta-analysis demonstrated there is no difference in the risk of hospital mortality, ICU length of stay (LOS) and nosocomial acquired infection between permissive underfeeding enteral nutrition and standard feeding under enough protein.

Permissive Underfeeding or Standard Enteral Feeding in Critically Ill Adults.

BACKGROUND The appropriate caloric goal for critically ill adults is unclear. We evaluated the effect of restriction of nonprotein calories (permissive underfeeding), as compared with standard enteral feeding, on 90-day mortality among critically ill adults, with maintenance of the full recommended amount of protein in both groups. METHODS At seven centers, we randomly assigned 894 critically...

Nutritional Assessment in Critically Ill Patients

Background: Malnutrition is an important factor in the survival of critically ill patients. The purpose of the present study was to assess the nutritional status of patients in the intensive care unit (ICU) on the days of admission and discharge via a detailed nutritional assessment. Methods: Totally, 125 patients were followed up from admission to discharge at 8ICUs in Shiraz, Iran. The patien...

Underfeeding versus full enteral feeding in critically ill patients with acute respiratory failure: a systematic review with meta-analysis of randomized controlled trials.

INTRODUCTION Although guidelines emphasize that the provision of enteral nutrition (EN) should be as close as the patient's needs, prospective studies question this strategy. OBJECTIVE To compare the effect of two EN strategies (underfeeding versus full-feeding) on ICU and overall mortality (hospital mortality or 60-day mortality) and length of stay (LOS), duration of mechanical ventilation (...

Permissive underfeeding versus target enteral feeding in adult critically ill patients (PermiT Trial): a study protocol of a multicenter randomized controlled trial

BACKGROUND Nutritional support is an essential part of the management of critically ill patients. However, optimal caloric intake has not been systematically evaluated. We aim to compare two strategies of enteral feeding: permissive underfeeding versus target feeding. METHOD/DESIGN This is an international multi-center randomized controlled trial in critically ill medical- surgical adult pati...

Permissive underfeeding and intensive insulin therapy in critically ill patients: a randomized controlled trial.

BACKGROUND Nutritional support has been recognized as an essential part of intensive care unit management. However, the appropriate caloric intake for critically ill patients remains ill defined. OBJECTIVE We examined the effect of permissive underfeeding compared with that of target feeding and of intensive insulin therapy (IIT) compared with that of conventional insulin therapy (CIT) on the...
